Introduction
Last updated 22 October 2026. Among commission-earning providers with a calendar-month continuing compounded tirzepatide price, the lowest is $129 a month at yourEra, the median is $299, and the range is $129 to $399 across 13 providers. SnagRX and Strut Health publish $149 a month. A prepaid rate is outside that range: SnagRX's page shows $119 a month on the 12-month column, and CoreAge Rx shows $129 a month on a 12-month plan paid in full. Found Health's page prices a 28-day cycle, and Enhance.MD's page bills every 4 weeks.
Those two rows stay inside the catalog count of 13 because the stored offers are not marked as 28-day periods. LillyDirect's FDA-approved Zepbound vial starts at $299 for 2.5 mg and is a different product. Compounded tirzepatide is not FDA-approved. The program page is the live comparison.
Three prices, and which one the median uses
Telehealth Ledger keeps three price bases apart. The first-month price is an introductory charge. The continuing price is the later calendar-month charge, with medication included and no prepaid commitment. The prepaid figure is a longer plan, quoted with its term. The median in the answer uses only the continuing price, one per provider.
On 22 October 2026 that set is 13 commission-earning providers, from $129 to $399, median $299. The middle of the sorted list is Shed Rx at $299. yourEra is the low. MEDGm and Willow are the high, both $399. The program page is the live table. This note is the cost comparison, and it links back there rather than replacing it.

Continuing rows, with the page interval named
The 13 rows below are the catalog's calendar-month continuing set on 22 October 2026. Where the provider's own page uses a 28-day cycle or a 4-week bill, the row says so. Those pages were opened the same day, except bmiMD, which returned HTTP 403, and MyDrHank, which returned no price. Those two keep the catalog figure and its check date.
Found Health and Enhance.MD are inside the 13. Found Health's page says every price is per 28-day cycle. Enhance.MD's page says the 1-month plan is billed every 4 weeks. A reader comparing a calendar-month charge with those two should use the interval on the page, not only the catalog count.

| bmiMD | $249 a month | The catalog also records $2,148 up front for 12 months. The page returned HTTP 403, so this row is the catalog figure. | 22 October 2026 |
| Shed Rx | $299 a month | The catalog also records $1,614 up front for 6 months and $2,868 up front for 12 months, and describes the one-month plan as month-to-month after a two-month minimum. The page does not print those lump sums. | 22 October 2026 |
| Enhance.MD | $329 in the catalog count | The page bills $329 every 4 weeks, with $50 off the first order ($279). It also lists $313, $296, and $280 a month on plans billed every 12, 24, and 48 weeks. Four weeks of medication are included. No pharmacy named. | 22 October 2026 |
| RxSpan MD | $329 a month | Public product API, price unit Month. Longer product records at $927, $1,734, and $3,228 use the same unit label, so they are not treated as monthly prices. No pharmacy name. | 22 October 2026 |

| Found Health | $359 in the catalog count | The page says prices are per 28-day cycle and include clinical care, medication, and membership. Cash is $219 the first cycle, then $359, or $309, $289, and $239 per cycle on 3, 6, and 12 months. Licensed 503A pharmacies, not named. The page describes compounded tirzepatide + B12/Glycine. compounded (not FDA-approved) | 22 October 2026 |
| MEDGm | $399 a month after $279 the first month | The page also shows 3 months at $897 ($299 a month), 6 months at $1,734 ($289 a month), and 12 months at $3,228 ($269 a month). The catalog stores $279 then $399. Medication, labs, consult, and shipping are described as included. | 22 October 2026 |
| Willow | $399 a month | Free 2-day shipping. Pharmacy-prepared, with state-licensed compounding pharmacy partners. The pharmacy is not named. | 22 October 2026 |
Found Health's cash page also lists an insurance track at $149 the first cycle and $289 after, or $239, $209, and $179 per cycle on 3, 6, and 12 months. It says the video visit is billed through insurance. Those figures are not the $359 cash row.
Prepaid, 4-week, and comparison-listing prices
Several published prices are real and still sit outside the median. CoreAge Rx shows $129 a month and says that figure is a starting price based on a 12-month plan paid in full. The page does not print $1,548. The catalog records that 12-month total, checked 22 October 2026. There is no separate calendar-month continuing price for CoreAge Rx, so $129 is not the low of the continuing range. yourEra's $129 is that low, and it is month-to-month.
Sprout Health's homepage bills tirzepatide at $199 every 4 weeks, and $149 a month on a 12-month plan paid as $1,788 up front. The catalog marks that offer as a 28-day period, checked 21 October 2026, so it is not one of the 13. The page says medication is prepared by licensed U.S. pharmacies and that Sprout is not a pharmacy. It does not name the pharmacy.
Embody's product page shows $129.00 and $349.00 for a one-month supply and calls $129 a sale that saves $220. It does not say the sale is limited to the first month. The catalog keeps $349 as the regular price and does not use $129 as the continuing price. Checked 21 October 2026.
GobyMeds and Affinity Direct appear in the public price table as comparison listings. Neither is a commission link, and this article does not use a referral code. Their pricing pages were not re-opened for this note. The catalog, checked 22 October 2026, records GobyMeds at $299 for one month and $399 up front for 3 months. The catalog, checked 21 October 2026, records Affinity Direct at $147.90 every 4 weeks for a 2.5 mg/week dose and $360.40 every 12 weeks. A 4-week bill is not the calendar-month low.

LillyDirect Zepbound vials are a different product
Lilly's Zepbound page on LillyDirect, opened 22 October 2026, lists vial self-pay from $299 to $449. On that page a month is 28 days and 4 single-dose vials. The 2.5 mg/0.5 mL vial starts at $299 a month, and the page calls 2.5 mg a starting dosage rather than a maintenance dose. The 5 mg/0.5 mL vial starts at $399. The 7.5 mg, 10 mg, 12.5 mg, and 15 mg vials, each 0.5 mL, start at $449 when the refill is within 45 days of the prior delivery. If the refill is not within 45 days, the same page lists $499 for 7.5 mg and $699 for 10 mg, 12.5 mg, and 15 mg.
The Zepbound terms page lists the same regular 1-month prices for a vial or a KwikPen: $299 at 2.5 mg, $399 at 5 mg, $499 at 7.5 mg, and $699 at 10 mg, 12.5 mg, and 15 mg. Self Pay Journey is $449 for each 1-month supply of the 7.5 mg, 10 mg, 12.5 mg, and 15 mg vial or KwikPen, for a first purchase beginning 23 February 2026, when the refill is within 45 days. Taxes and fees may apply. The terms define a month as 28 days and 4 single-dose vials, or one KwikPen. That 28-day month is not a calendar-month telehealth charge.
Zepbound is FDA-approved. Compounded tirzepatide is not FDA-approved, and a catalog price is not a claim that the compounded product is the Lilly product. The longer Zepbound comparison is on the existing compounded-tirzepatide and Zepbound article. Multi-month compounded plans are on the 3, 6, and 12 month note. A first-month discount that rises later is explained on the month-one price article. Split-pay statements are on the financing page.
Compounded drugs are not FDA-approved. FDA says a compounded drug should be used only when a patient's medical needs cannot be met by an FDA-approved drug.
What the price does not name
A published monthly price does not name the pharmacy that would fill a prescription. SnagRX's tirzepatide page and Embody's tirzepatide page list Red Rock Compounding Pharmacy in St. George, Utah; Health Warehouse in Florence, Kentucky; Precision Medicine in Bellmore, New York; and Triad Rx in Daphne, Alabama. MEDGm's terms spell the Utah pharmacy RedRock Pharmacy and list the same three other firms. None of those names is Houston Rx LLC. That is a statement about the lists, not a recall clearance.
yourEra, Strut Health, PlexusDx, HealthRX, Found Health, Willow, CoreAge Rx, Enhance.MD, RxSpan MD, and Sprout Health do not name a dispensing pharmacy on the tirzepatide page opened 22 October 2026. bmiMD returned HTTP 403. MyDrHank's product page returned no pharmacy text. GobyMeds and Affinity Direct were not re-opened, so this note does not say what their pages claim. Ask the program which pharmacy fills the prescription, then read the name on the label.
FDA's Enforcement Report event 99990, a separate note, lists a Class II recall by Houston Rx LLC of 64 tirzepatide/niacinamide vials. The 14 October 2026 openFDA file used for the Carolina Infusion and Apothecary Pharma note does not include that event. Neither record is a finding that a provider in the table above dispensed a recalled lot.
This comparison does not say a particular vial is appropriate to use. Questions about a label go to the dispensing pharmacy or the prescriber.
Common questions
- How much does compounded tirzepatide cost in 2026?
- On 22 October 2026, 13 commission-earning providers have a calendar-month continuing price. The lowest is $129 at yourEra, the median is $299, and the highest is $399. Prepaid columns, a 12-month paid-in-full price, and 4-week bills are outside that range.
- Why is $119 not the lowest continuing price?
- SnagRX's page shows $119 a month on the 12-month column, next to $149 for one month. The continuing comparison uses $149. CoreAge Rx's $129 is a starting price on a 12-month plan paid in full, not a separate month-to-month charge. yourEra's $129 month-to-month price is the low of the continuing range.
- Do Found Health and Enhance.MD bill a calendar month?
- The catalog counts both in the 13, at $359 and $329. Found Health's page says every listed price is per 28-day cycle. Enhance.MD's page says the 1-month plan is billed every 4 weeks. Those page intervals are the ones to use when comparing a calendar-month charge.
- How does that compare with LillyDirect Zepbound?
- Lilly's vial page, opened 22 October 2026, starts at $299 for a 2.5 mg self-pay month, defined as 28 days and 4 vials. Higher strengths are $399 or, within 45 days, $449, and the terms list $499 and $699 when that window is missed. Zepbound is FDA-approved. Compounded tirzepatide is not.
- Which providers name the pharmacy?
- On 22 October 2026, SnagRX and Embody name Red Rock Compounding Pharmacy, Health Warehouse, Precision Medicine, and Triad Rx. MEDGm's terms name RedRock Pharmacy and the same three other firms. None of those names is Houston Rx LLC. The other opened commission-earning tirzepatide pages do not name a pharmacy. bmiMD and MyDrHank could not be read for one.
- Is compounded tirzepatide FDA-approved?
- No. FDA's concerns page says compounded drugs are not FDA-approved and should be used only when a patient's medical needs cannot be met by an FDA-approved drug. yourEra and Strut Health say the same about the products on their pages. A lower monthly price is not a statement about FDA status.
Use the continuing monthly price, then read the plan term
The calendar-month compounded tirzepatide range on 22 October 2026 is $129 to $399, median $299, across 13 commission-earning providers. yourEra is $129 month-to-month. SnagRX's $119 column, CoreAge Rx's $129 paid-in-full price, Sprout Health's 4-week bill, and Embody's $129 sale are different charges. Found Health and Enhance.MD are inside the catalog count and, on their pages, are a 28-day cycle and a 4-week bill. The program page is the live comparison.
