LillyDirect vs telehealth Zepbound price: the ladder and what sits on top of it

Zepbound self-pay pricing is a dose ladder published by Lilly, not a menu each telehealth program writes. The table shows the KwikPen prices from Lilly's savings page, the regular prices that apply outside the 45-day refill window, and the single-dose vial prices Lilly announced in December 2025 as reported in the trade press. A telehealth program's Zepbound price is one of these numbers plus whatever the program charges for its clinician; the catalog module below shows the current program figures.

How the 45-day rule works

Lilly's savings page states the condition in full. To pay as low as $449 for a one-month fill of Zepbound 7.5 mg, 10 mg, 12.5 mg, or 15 mg KwikPen, you must complete your refill purchase within 45 days of the delivery or received date of your previous Zepbound KwikPen prescription fill. If the refill purchase is completed more than 45 days after that delivery, the purchase offer will not apply, and the regular price of $499 for 7.5 mg or $699 for 10 mg and above is charged for that fill.

The clock runs from delivery, not from your last injection. A four-dose KwikPen covers four weeks, so a patient who refills as each pen runs out stays comfortably inside 45 days. A patient who pauses for a month, or whose telehealth program delays the renewal prescription, falls outside it and pays $50 to $250 more for the next fill. The lower doses are unaffected; 2.5 mg and 5 mg cost $299 and $399 regardless of timing.

What a telehealth Zepbound price adds

A telehealth program's role in a Zepbound purchase is the prescription and the follow-up. Lilly's provider page states that all licensed healthcare providers have access to the LillyDirect EHR listings and can send prescription requests, and lists LillyDirect Pharmacy at NPI 1912889320. Once the prescription arrives, LillyDirect texts the patient to confirm details within one business day and sends a checkout link within two business days of a status update. The patient pays LillyDirect for the pen or vials, and the program separately for its membership.

Ro's pricing page, for example, lists the Zepbound KwikPen at Lilly's starting dose price and states that the cost of medication is not included in the Ro Body membership fee. That is the structure to expect: the manufacturer's ladder, plus a fee. A program that quotes a single Zepbound price higher than Lilly's tier for your dose is bundling its fee into the number; a program that quotes a lower one is either describing an insurance route or leaving something out.

Vials, KwikPens, and single-dose pens: which channel sells which

Lilly Medical states that Zepbound vials and multi-dose KwikPens are available in all strengths exclusively through LillyDirect's self-pay pharmacy channel, with a valid electronic prescription from the health care provider of the patient's choice. Single-dose pens are the presentation that goes through insurance pharmacies and LillyDirect's insurance component. The label lists all three: single-dose pens and vials in 2.5 mg to 15 mg per 0.5 mL, and multi-dose KwikPens delivering four doses at each strength.

For a self-pay patient that means the vial and the KwikPen are the products on the table, both priced on the same ladder, and both only from LillyDirect. A telehealth program cannot fill a Zepbound vial or KwikPen prescription from its own partner pharmacy. Insurance, the savings card, and the Medicare GLP-1 Bridge

Two routes beat the self-pay ladder, and both apply to the single-dose pen rather than the self-pay products. Lilly's savings card lets commercially insured patients pay as little as $25 for a one-, two-, or three-month fill of the Zepbound single-dose pen, with a maximum annual saving of $1,300 per calendar year; the card expires December 31, 2026. Government-program enrollees are excluded.

Medicare is the other route. CMS states that eligible Part D beneficiaries pay a $50 copay under the Medicare GLP-1 Bridge from July 1, 2026 through December 31, 2027, that the Zepbound KwikPen formulation is covered, and that the single-dose vial and single-dose pen formulations of Zepbound will not be available through the Bridge. Lilly's provider page repeats that the Bridge applies to the KwikPen only. Neither route depends on the prescriber being a telehealth clinician.

Sending a telehealth prescription to LillyDirect

The steps are the same for a telehealth prescriber and a local one, and the questions to ask the program come first.
