Why did my price go up after the first month? The four mechanisms

Every legitimate month-two increase traces to one of four rules, and each rule is published somewhere before you pay. The table names them, says who sets them, and shows where the wording lives. A fifth case, a charge you never agreed to, is not a mechanism; it is a violation, and it gets its own section below.

Introductory membership rates end

Membership-model programs commonly discount the first month to get you through intake. Ro's pricing page advertises a discounted first month, describes the ongoing membership as monthly or prepaid annually at a lower effective rate, and states separately that the cost of medication is not included in the membership fee. The month-two increase there is two things at once: the membership returning to its standard rate and, if you were prescribed, the first full medication charge landing.

Programs that bill on a fixed cycle can create the same surprise without a promotion. Wellorithm's terms state that membership fees are billed every 28 days and are non-refundable once processed, so a second charge can arrive before a calendar month has passed. Found's offer terms make the membership fee refundable only within three days of purchase or before the first medical consultation, which means the point at which you can still back out comes early.

Manufacturer introductory offers end: Wegovy $199, then $349

Novo Nordisk runs the clearest example. Its NovoCare Pharmacy page states that patients new to the Wegovy Savings Offer and NovoCare Pharmacy pay $199 for each month of 0.25 mg and 0.5 mg for two monthly fills through December 31, 2026. After that, the page lists $349 per month for Wegovy 0.25 mg, 0.5 mg, 1 mg, 1.7 mg, or 2.4 mg and $399 per month for Wegovy HD 7.2 mg. The Wegovy pill is priced by dose from the start: $149 for 1.5 mg, $199 for 4 mg, and $299 for 9 mg and 25 mg.

So a patient who starts Wegovy pens in September pays $199 twice and $349 from the third fill, and a telehealth program that routes the prescription to NovoCare passes that exact step along. The increase is not a program decision, and the label's escalation schedule means the third fill is usually also the first 1 mg fill, which makes the jump feel like a dose charge even though every pen dose costs the same at NovoCare.

Dose-ladder pricing: Zepbound $299, $399, $449, and the 45-day rule

Lilly prices Zepbound by strength. Its savings page lists the KwikPen self-pay price through LillyDirect at $299 for 2.5 mg, $399 for 5 mg, and as low as $449 for 7.5 mg, 10 mg, 12.5 mg, and 15 mg. The Zepbound label starts every patient at 2.5 mg for four weeks and moves to 5 mg after that, so the second month costs $100 more by design, and the third or fourth month, at 7.5 mg or higher, costs $150 more than the first.

The $449 tier carries a condition. Lilly states that the purchase offer applies only if you complete your refill purchase of the Zepbound KwikPen within 45 days of the delivery or received date of your previous KwikPen fill. If the refill is completed more than 45 days after that delivery, the offer does not apply and the regular price does: $499 for 7.5 mg and $699 for 10 mg, 12.5 mg, and 15 mg. A skipped month therefore raises the next month's price by $50 to $250 until the 45-day rhythm is re-established. Lilly's December 2025 announcement, reported in the trade press, set single-dose vials on the same $299, $399, and $449 ladder.

Prepaid discounts revert if you leave early

Multi-month plans quote an effective monthly price that only holds if you finish the term. Ageless states it directly: multi-month discounts are conditional on completing the selected plan term, and if you cancel early after receiving any medication, consultation, or other plan benefit, the amount already supplied is repriced using the shorter-duration plan pricing that was available when you purchased. Its terms add that medication and fulfillment charges are non-refundable once a prescription or order has been submitted to a pharmacy.

Other programs handle the unused months differently. TelePep refunds the pro-rated value of any months not yet shipped if you cancel before the term ends, less an administrative fee. Embody states that all payments for the subscription services are final and non-refundable at the time of purchase. In every case the number to compare is the price per shipped month if you stop after one, not the advertised effective rate.

When a price increase is not legitimate

A price that rises because of a rule you were shown is a disclosure. A price that rises because of a rule you were not shown is the conduct the FTC pursued against NextMed, whose December 3, 2025 final order cites undisclosed costs and membership commitments and charging consumers without express informed consent. The Restore Online Shoppers' Confidence Act requires an online seller to disclose clearly all material terms of the transaction and to obtain the consumer's express informed consent before charging a recurring fee. A pending complaint against Hims & Hers, filed July 29, 2026 by the FTC, Utah, and Los Angeles County, alleges deceptive billing and cancellation practices; no finding has been made.

If your month-two charge does not match any rule on the page you saw at checkout, keep the checkout page, the confirmation email, and the charge, and ask the program in writing which term authorizes the amount. If the answer is missing, the dispute path runs through your card issuer, your state attorney general, and the FTC's complaint site.

How to check what month two will cost before paying

Four questions, asked before the first charge, remove the surprise.

• Is the first month a promotional rate, and what is the standard membership after it?
• Is the medication billed separately, and what does it cost at the dose the label says you reach in month two and month three?
• If the drug is Zepbound, does the program route the prescription to LillyDirect, and does its refill cadence keep you inside the 45-day window?
• If the plan is prepaid, what happens to the discount and the unshipped months if you stop after one shipment?
