Introduction
Foundayo (orforglipron) is Lilly's once-daily small-molecule oral GLP-1, approved on April 1, 2026 for long-term weight reduction in adults with obesity, or overweight with a weight-related condition. Lilly's self-pay price is $149 for 0.8 mg, $149 for 2.5 mg and $199 for 5.5 mg on temporary cuts that end December 31, 2026, and $299 for 9 mg.
The 14.5 mg and 17.2 mg doses are $299 when each refill is completed within 45 days, and $349 otherwise. A month is 30 days, so 12 fills at $299 equal $3,588 over 360 days.
Self-pay price by dose
Lilly publishes a self-pay price for each Foundayo tablet strength. A 1-month supply is 30 days. Additional taxes and fees may apply, and Lilly can change or end the offers at any time. The July 2026 label lists tablets of 0.8, 2.5, 5.5, 9, 14.5, and 17.2 mg in bottles of 30. The labeled starting dosage is 0.8 mg once daily, then 2.5 mg and 5.5 mg after at least 30 days at each step, then 9 mg, 14.5 mg, or 17.2 mg after at least 30 days, with a maximum of 17.2 mg. The price row follows the strength on the prescription. Those steps are label facts for a prescriber.
The 0.8 mg price of $149 and the 9 mg price of $299 match the regular prices Lilly lists beside the temporary cuts. The 2.5 mg price of $149 and the 5.5 mg price of $199 are temporary through December 31, 2026. New and existing patients are eligible for the 14.5 mg and 17.2 mg purchase offer, and it applies automatically on the first purchase. The 9 mg terms list $299 without a 45-day condition.
| Dose | Self-pay price | Condition |
|---|---|---|
| 0.8 mg | $149 | Regular price is also $149 |
| 2.5 mg | $149 | Temporary through December 31, 2026; regular price $199 |
| 5.5 mg | $199 | Temporary through December 31, 2026; regular price $299 |
| 9 mg | $299 | Regular price is also $299 |
| 14.5 mg | $299 or $349 | $299 if each refill is within 45 days; otherwise $349 |
| 17.2 mg | $299 or $349 | Same 45-day purchase offer as 14.5 mg |
What 12 fills cost
Because Lilly defines a month as 30 days, 12 fills are 12 x 30 = 360 days, rather than a 365-day calendar year. At the regular 0.8 mg price, 12 x $149 = $1,788. At 9 mg, 12 x $299 = $3,588. The regular 2.5 mg price is $199, so 12 x $199 = $2,388. The regular 5.5 mg price is $299, so 12 x $299 = $3,588.
At 14.5 mg or 17.2 mg, refills completed inside 45 days are 12 x $299 = $3,588. Refills outside that window are 12 x $349 = $4,188. The gap is 12 x ($349 - $299) = 12 x $50 = $600. The temporary 2.5 mg and 5.5 mg prices end December 31, 2026, so the lower temporary figures are not a 12-fill schedule Lilly has published past that date.
The 45-day refill rule for 14.5 mg and 17.2 mg
The $299 purchase offer for 14.5 mg and 17.2 mg continues when the next fill is completed within 45 days of delivery or receipt of the previous fill. If the refill is completed more than 45 days after that delivery or receipt, the purchase offer does not apply and the regular price is $349. The terms use delivery or receipt, not the date the prescription was written.
If Lilly cannot verify the 45-day refill at the point of sale, a post-transaction reimbursement claim is possible for the difference between the amount paid and the amount the offer would have charged. Claims are due by March 31, 2027. The first purchase receives the $299 offer automatically, for new and existing patients.
Prices that change on December 31, 2026
Two dose prices are marked temporary. Through December 31, 2026, 2.5 mg is $149 instead of the regular $199, and 5.5 mg is $199 instead of the regular $299. After that date, the regular prices Lilly already lists are the published figures for those strengths. The 0.8 mg price stays $149 in both columns, and the 9 mg price stays $299.
The commercial savings card and the self-pay savings card both expire, and their savings end, on December 31, 2026. TrumpRx shows the same temporary 2.5 mg and 5.5 mg reductions through that date. The 14.5 mg and 17.2 mg $299 offer is a purchase offer Lilly can change or end at any time. The terms do not give that offer the same December 31, 2026 end date written for the 2.5 mg and 5.5 mg cuts.
Savings card caps for commercial insurance
When commercial drug insurance covers Foundayo, the card can bring a 1-month (30 days), 2-month (60 days), or 3-month (90 days) fill to as little as $25. Maximum savings are $100, $200, or $300 for those fill lengths, $1,000 per calendar year, and at most 10 fills per calendar year. The card cannot be combined with other Foundayo offers.
Eligibility is commercial drug insurance, no enrollment in a government program such as Medicaid, Medicare (including Part D and Medicare Advantage), DoD, VA, or TRICARE, residence in the United States or Puerto Rico, and age 18 or older. Lilly's terms also exclude a plan that requires the card through an alternate funding program as a condition of coverage.
When commercial insurance does not cover Foundayo, the card lists as low as $149 for 0.8 mg (maximum monthly savings $520), $149 for 2.5 mg (maximum $520), $199 for 5.5 mg (maximum $470), and $299 for 9 mg, 14.5 mg, and 17.2 mg (maximum $370). The same card allows up to 10 fills a year and expires December 31, 2026. Lilly's separate self-pay savings card lists the same dose prices and the same maximum monthly savings for cash patients who do not seek insurance reimbursement. Savings on 2.5 mg, 5.5 mg, 14.5 mg, and 17.2 mg follow the purchase-offer terms.
List price and the TrumpRx figure
Lilly's pricing information page lists a wholesale acquisition cost of $649.00 per month for a 30-day supply. That list price sits above every self-pay dose price in the table. Subtracting the default self-pay price gives $649.00 - $149 = $500.00, the savings amount TrumpRx prints on the default 0.8 mg strength.
TrumpRx shows an original price of $649.00, savings of $500.00, a price of $149, and the phrase "up to 77% off." It also shows the temporary 2.5 mg and 5.5 mg reductions through December 31, 2026, and it sends buyers to LillyDirect. TrumpRx is the same Lilly self-pay price, not an extra discount stacked on LillyDirect.
Medicare's $50 GLP-1 Bridge
Starting July 1, 2026, the temporary Medicare GLP-1 Bridge covers Foundayo tablets for eligible Part D enrollees at a $50 copayment for a one-month supply of 28 or 30 days. The same page lists Wegovy, injection or tablet, and Zepbound KwikPen only. The $50 does not count toward the Part D deductible or out-of-pocket limit, Extra Help cannot lower it, and it does not appear on the Part D Explanation of Benefits.
A person must be 18 or older and have a BMI of 35 or higher, or a BMI of 30 or higher with diastolic heart failure, uncontrolled hypertension, or chronic kidney disease stage 3a or higher, or a BMI of 27 or higher with prediabetes, a prior heart attack or stroke, or symptomatic peripheral artery disease. The page says someone is not eligible who already gets GLP-1s covered by a Part D plan, or who has type 2 diabetes, moderate-to-severe sleep apnea, or fatty liver disease. The provider sends the prescription and, when requested, a prior authorization, and must certify use as part of a lifestyle program focused on diet and exercise. Prior authorization stays valid through December 31, 2027 unless the GLP-1 changes.
Membership is separate, and the label carries a boxed warning
Shed Rx sells Foundayo and bills the medication separately from the Shed membership. The current membership and medication figures are on the Shed Rx provider page, which is the record to read before adding a clinic fee to Lilly's dose price. This page keeps those provider figures out of the manufacturer table.
A telehealth total can therefore have two parts: Lilly's price for the prescribed strength, and a separate membership or visit charge. FDA approved Foundayo on April 1, 2026, 50 days after filing, as the first new molecular entity under the Commissioner's National Priority Voucher pilot. Lilly's April 9, 2026 announcement says the medicine is available through LillyDirect, telehealth providers, and U.S. retail pharmacies. The July 2026 label carries a boxed warning for thyroid C-cell tumors and says Foundayo is not for patients with a personal or family history of medullary thyroid carcinoma or MEN 2. Lilly's approval announcement lists nausea, constipation, diarrhea, vomiting, indigestion, stomach pain, headache, swollen belly, feeling tired, belching, heartburn, gas, and hair loss as the most common side effects, and says oral birth control may work less well, with a provider possibly recommending another method for 30 days after the start and after each dose increase.
Common questions
- How much does Foundayo cost per month without insurance?
- Lilly's self-pay price, checked October 15, 2026, is $149 for 0.8 mg, $149 for 2.5 mg through December 31, 2026, $199 for 5.5 mg through that date, and $299 for 9 mg. The 14.5 mg and 17.2 mg doses are $299 when each refill is completed within 45 days, and $349 otherwise. A month is 30 days. Taxes and fees may be extra.
- What is the 45-day refill rule?
- For 14.5 mg and 17.2 mg, the $299 purchase offer continues only when the next fill is completed within 45 days of delivery or receipt of the previous fill. A later refill uses the $349 regular price. If Lilly cannot verify the timing at the register, a reimbursement claim is due by March 31, 2027.
- Is TrumpRx cheaper than LillyDirect?
- No. TrumpRx shows an original price of $649.00 and savings of $500.00, which is $149, the same self-pay price Lilly lists for the default 0.8 mg strength. The page routes buyers to LillyDirect and shows the same temporary 2.5 mg and 5.5 mg cuts through December 31, 2026.
- What does Medicare charge for Foundayo?
- Eligible Part D enrollees in the Medicare GLP-1 Bridge pay $50 for a one-month supply of 28 or 30 days. The Bridge started July 1, 2026. The $50 does not count toward the Part D deductible or out-of-pocket limit, and Extra Help cannot lower it. People who already have GLP-1 coverage through Part D, or who have type 2 diabetes, moderate-to-severe sleep apnea, or fatty liver disease, are outside the Bridge.
- Can someone with Medicare use the Foundayo savings card?
- The commercial Foundayo Savings Card excludes people enrolled in Medicare, including Part D and Medicare Advantage, and other government programs. Lilly's separate self-pay savings card does not list that exclusion. It is for cash-paying patients who agree not to seek reimbursement from any insurer or state or federal program and not to count the cost toward a deductible or true out-of-pocket requirement. Eligible Bridge enrollees can instead pay the $50 Bridge copay. Both cards expire December 31, 2026.
- How much is 12 fills at 9 mg, or at 17.2 mg?
- At 9 mg, 12 x $299 = $3,588. At 17.2 mg, 12 x $299 = $3,588 when every refill stays inside 45 days, and 12 x $349 = $4,188 when they do not. Twelve fills are 360 days because a month is 30 days.
Match the dose, the refill clock, and the payer
The self-pay answer is a dose table: $149, $199, $299, or $349, with the 14.5 mg and 17.2 mg row depending on a 45-day refill and the 2.5 mg and 5.5 mg cuts ending December 31, 2026. Twelve on-time fills at $299 are 12 x $299 = $3,588 over 360 days. The savings card, TrumpRx, and the Medicare $50 Bridge are separate routes, and a telehealth membership is a separate bill from the medication.
