Strive Pharmacy
Strive Pharmacy LLC, 1275 E Baseline Rd, Suite 104, Gilbert, Arizona. Public-record profile of a pharmacy that fills telehealth prescriptions; records checked 10 September 2026.
Which telehealth providers use Strive Pharmacy
2 telehealth providers in the Telehealth Ledger directory name Strive Pharmacy as a fulfillment pharmacy on their own public pages: Bodybuilding Health+ and Pallas Health. Each name below links to that provider's profile, where the pharmacy statement sits beside its dated source.
Bodybuilding Health+'s pharmacy disclosure, as recorded in its dossier: The site names Belmar, Strive, Epiq Scripts, and Casa Pharma Rx.
Naming a pharmacy does not mean every prescription goes there. Most telehealth services list several partners and route each order by medication, state licensure, and stock, so the pharmacy on your label can differ from the marketing page. The vial label is the authoritative record of who compounded and dispensed it.
503A vs 503B compounding pharmacy: what it means for your prescription
Strive Pharmacy is recorded here as a 503A compounding pharmacy. Its telehealth-partner page says Strive is a licensed 503A compounding pharmacy and claims active pharmacy licenses in all 50 states; Strive Pharmacy LLC does not appear on FDA's registered outsourcing facility list (checked 2026-09-10).
A 503A pharmacy compounds a patient-specific prescription and is licensed and inspected primarily by its state board of pharmacy; it does not register with FDA as an outsourcing facility and is not required to follow federal current good manufacturing practice (CGMP). Compounded semaglutide or tirzepatide from a 503A pharmacy is not an FDA-approved product, and the beyond-use date on the label comes from the pharmacy's own stability practice under USP standards rather than from an approved drug application.
FDA inspection history and public regulatory records
3 public regulatory records for Strive Pharmacy were found in the databases checked on 10 September 2026. Each is summarized neutrally below, in date order, with a link to the primary document. A record is a fact about a filing, not a judgment about current practice: a Form 483 lists an inspector's observations, a warning letter states FDA's position after review, and a close-out letter records that FDA considers the letter's issues addressed.
Databases checked: FDA registered outsourcing facilities list (updated 2026-09-08) (no record found); FDA compounding inspections, recalls, and other actions (record found).
If a new letter, action, or recall appears, it is added with its date and primary link and earlier summaries are not rewritten.

- 13 May 2026 · FDA: Warning letter close-out to Strive Pharmacy Texas LLC dba Strive Pharmacy. FDA wrote that the firm appeared to have adequately addressed the violations cited in the 2022 warning letter. Primary document
State pharmacy license and accreditation
A public Arizona board of pharmacy license record for Strive Pharmacy LLC was not found through the lookups tried, or the lookup requires an interactive search that cannot be linked directly. That does not mean the pharmacy is unlicensed; most state verification portals do not expose stable URLs. Non-resident licenses in the states it ships to are held in each of those states' own systems.
Accreditation statement, in the pharmacy's own words: "NABP, PCAB/ACHC, and LegitScript accreditations in addition to the standard compliance floor."
Beyond-use date (BUD) and testing policy
Strive Pharmacy publishes this about stability, testing, or beyond-use dating: "Frequently testing active ingredients and finished compounded products via third-party analytical assessment for sterility, potency, and endotoxins."
A beyond-use date (BUD) is the date after which a compounded preparation must not be used; the pharmacy assigns it under USP <797> sterile compounding standards and prints it on the dispensing label. It is not the same as a manufacturer's expiration date on a brand pen. Follow the storage and BUD printed on your pharmacy label; if a vial arrives without one, ask the dispensing pharmacy before use.
How compounded GLP-1 medication is shipped from this pharmacy
Strive Pharmacy describes its shipping this way: "It has been shipped on ice for transit, but it's best to remove it from its shipping container and place it in the refrigerator immediately upon arrival."
Provider shipping claims: Bodybuilding Health+ says Shipping is described as discreet but delivery timing is not guaranteed. Pallas Health says Pallas describes free discreet delivery, commonly about one week after approval.
Compounded semaglutide and tirzepatide ship refrigerated: an insulated box with cold packs, usually by an overnight or two-day carrier service on weekdays so a package is never in transit over a weekend. On arrival, put the vial in the refrigerator, check the label for the BUD and storage line, and contact the pharmacy if the cold pack is fully thawed or the vial is warm.
Verification notes
- The 2022 warning letter and 2023 Form 483 concern the Boerne, Texas site operated by Strive Pharmacy Texas LLC, formerly Vita Pharmacy, LLC dba Talon Pharmacy of Boerne; the 2026 close-out was addressed to Strive's Gilbert, Arizona headquarters. No FDA action naming the Gilbert site itself was found.
- Arizona license Y007406 appears on an NPI-derived directory only and is not recorded as verified; the Arizona board lookup was not fetched.
- Strive's accreditation statement is the pharmacy's own; the NABP directory did not render. Its shipping policy says processing can take up to 3 days, and its partner page says temperature-sensitive compounds ship with validated cold-chain packaging.
- Strive announced on 2026-01-14 that it filed an antitrust suit against Eli Lilly and Novo Nordisk. Eli Lilly's April 2025 suit against Strive and Empower over compounded tirzepatide marketing, and Arizona Board of Pharmacy matters reported by Sherwood News in February 2026, come from secondary reporting and are not recorded as verified.
- Strive announced a 350,000 square foot headquarters in Mesa, Arizona, in December 2025.
