Switching GLP-1 telehealth providers: what moves and what stays

A GLP-1 telehealth subscription bundles four things: a clinician relationship, a prescription, a pharmacy, and a payment plan. Switching providers separates them, and each one behaves differently depending on whether the drug is compounded or brand. The table is the short version; the sections after it explain each row with the source it rests on.

Why a compounded prescription stays with the old provider

A compounded GLP-1 product is prepared for one prescription by one pharmacy. FDA's consumer guidance is to obtain a prescription from a licensed doctor and fill it at a state-licensed pharmacy, and the 503A rules the pharmacy operates under tie each preparation to a patient-specific order. The old program's clinician wrote that order to the old program's partner pharmacy. A new program has its own clinicians and its own pharmacy, so it cannot simply take over the existing order; it evaluates you, writes a new prescription, and sends it to its own partner.

That matters for what arrives. The new pharmacy may ship a different concentration, a different vial size, or a different additive, and FDA's September 2026 statement lists dosing errors with compounded injectables as a source of adverse-event reports, some requiring hospitalization. FDA's telehealth guidance also bars branding that implies the telehealth company is the compounder, which is why asking the new program to name its pharmacy before you cancel the old one is the single most useful step.

Vials you already have follow the old pharmacy's instructions. FDA advises discarding a multi-dose vial within 28 days after the first use even if medication remains, so a switch that leaves a partly used vial behind does not stretch its life.

Switching brand Wegovy or Zepbound: the prescription can go to the manufacturer pharmacy

Brand drugs are different because the pharmacy does not belong to the telehealth program. NovoCare's Wegovy page tells patients to have their prescriber send the prescription to NovoCare Pharmacy, describes free home delivery, and allows a transfer for pickup at a local pharmacy. Sesame Care's Wegovy page says the same from the program side: ask your Sesame provider to send your prescription to the NovoCare pharmacy for the cash price.

Lilly's provider page states that all licensed healthcare providers have access to the LillyDirect EHR listings and can send prescription requests, and lists LillyDirect Pharmacy's NPI as 1912889320. Lilly Medical adds that Zepbound vials and multi-dose KwikPens are available in all strengths exclusively through LillyDirect's self-pay pharmacy channel with an electronic prescription from the health care provider of the patient's choice. So a Zepbound patient who leaves one telehealth program for another, or for a local clinician, keeps the same pharmacy and the same product; only the prescriber changes.

The one thing to confirm is whether the new program will send the prescription where you want it. Some programs only ship from their own pharmacy partner. Ask before paying a membership.

What happens to prepaid months and the current billing cycle

The money is where switching costs people. Programs publish their answer, and the answers differ widely. Ageless states that multi-month discounts are conditional on completing the selected plan term and that early cancellation reprices the medication already supplied at the shorter-plan rate; its terms also say cancellation prevents future renewals but does not reverse a charge or fulfillment step that was already completed. TelePep refunds the pro-rated value of any months not yet shipped if you cancel before the term ends, less an administrative fee.

Several programs make the current cycle final. Embody states that all payments for the subscription services are final and non-refundable at the time of purchase. Telos RX says cancellation will take effect at the end of the current billing cycle and any fees paid prior are non-refundable. Pallas Health says canceling takes effect at the end of your current billing period and, once a prescription is issued, the current cycle is not prorated or refunded. CoreAge Rx says cancelling stops future automatic renewals but does not automatically cancel or refund an order that has already been processed.

Timing the switch so you are not paying two programs

Cancellation notice periods decide whether the old program bills one more time. Embody and SnagRX both require the request at least 72 hours before the next billing date. Wisp requires cancellation at least 48 hours before the first day of the refill or renewal period. Strut Health states it cannot refund cancellation requests received after the pharmacy has processed the order. Direct Meds allows an attempt to cancel a prescription order within 24 hours of placing it, if the provider network has not yet received it.

The sequence that avoids a double charge is to complete the new program's intake and get its pharmacy named first, confirm the new prescription has been sent, then cancel the old program in writing before its notice deadline, and keep the confirmation. Cancelling first leaves a gap; cancelling late leaves an extra charge.

Dose continuity is the new prescriber's decision

Wegovy and Zepbound come in fixed strengths listed on their labels, and both labels describe an escalation schedule starting at the lowest dose. Compounded products come in whatever concentration the pharmacy prepares, and FDA notes that some contain salt forms that are different active ingredients from the approved drugs. There is no official conversion between a compounded vial and a brand pen, or between two pharmacies' vials.

Telehealth Ledger does not publish dose or syringe conversions. The new prescriber decides where you start based on what you were taking, how long, and the product now being prescribed. Bring the old pharmacy label and your dosing history to the new intake.

Do not carry a dose from one product to another on your own. Brand strengths, compounded concentrations, and salt forms differ, and the new prescriber has to set the starting dose.

A switching checklist

Work through these in order. Each step protects against one of the losses described above.

• Read the old program's cancellation notice period and prepaid-plan terms and note the exact deadline.
• Ask the new program which pharmacy fills its prescriptions and, for brand drugs, whether it will send the prescription to NovoCare Pharmacy or LillyDirect.
• Complete the new intake with your current pharmacy label and dosing history so the new prescriber can decide the starting dose.
• Confirm the new prescription has been transmitted before cancelling the old subscription in writing.
• Keep the old program's cancellation confirmation and watch for a further charge; dispute anything that no published term authorizes.
